Behavioral Health Care Coordinator

The Behavioral Health Care Coordinator (BHCC) works in collaboration and continuous partnership with chronically ill or high-risk mental health patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach. As a BHCC, you will be responsible for performing a variety of clerical functions to ensure an efficient operation and to enhance patient experience. This role would be responsible for scheduling initial and follow-up visits with a Behavioral Health provider, reminder appointment calls, insurance eligibility, Behavioral Health referrals, patient check-in and check-out and other related duties.

Case Management

Case Management / Patient Navigation

The Case Manager/Peer Navigator works to enhance the quality of patient management and maximize satisfaction. The successful candidate will be accountable for the full patient care cycle. Assisting Behavioral Health and Addiction Medicine providers in identifying and providing related resources for their patient by maintaining a current, thorough knowledge of community resources and utilize these to facilitate comprehensive service support to patients. The Case Manager/Peer Navigator works cooperatively and cohesively as a multidisciplinary team with other community service partners. Duties include but are not limited to assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed.

MAT Navigators (Medication - Assisted Treatment)

The MAT Navigator works in collaboration and continuous partnership with chronically ill or “high-risk” patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach. The MAT Navigator plays a pivitol role in supporting the Addiction Medicine provider with a primary focus on outreach services. As a MAT Navigator, you will facilitate SUD services, raise Substance Abuse awareness to school and the under-serve communities, increase comprehension through culturally and linguistically appropriate MAT education, Develop and strengthen community partnership, and Serve as the contact point, advocate, and informational resource for MAT services and expansion.
